ORGANIZATION OF SPECIALIZED MEDICAL CARE FOR PATIENTS WITH CHRONIC HEART FAILURE

Relevance. Data from epidemiological studies indicate an increase in the number of patients with chronic heart failure (CHF) in the Republic of Uzbekistan in recent years. Deterioration of the condition of patients with CHF after an episode of acute decompensation of heart failure (ADHF) requires a change in the tactics of treatment and rehabilitation of patients at the outpatient stage. Target. To determine the differences between the two strategies for monitoring patients with CHF after ADHF and to determine the effectiveness of treatment, rehabilitation measures and life prognosis based on observation in a specialized center for the treatment of CHF and the outpatient practice system. Materials and methods. The study involved 556 patients over 18 years of age with CHF of any etiology. The first group consisted of 312 patients suffering from CHF who were observed in the CHF hospital of the RSSPTSKMSF and continued their rehabilitation at the CHF Center; 2nd - 244 patients with CHF after discharge from the CHF department of ADHF were observed in the 1st family polyclinic on an outpatient basis in Samarkand. Results. After a year of observation, the overall mortality in the 1st group was 4.13%, in the 2nd - 14.83%. Physical activity and rehabilitation potential of patients observed in the city department of CHF were significantly higher than in patients treated in polyclinics. Conclusion. Changing the organization of medical care for patients with CHF leads to an improvement in the prognosis of patients and significantly increases the rehabilitation potential.
