Mitigating AI Hallucinations in the Search Spam Era: A Human–AI Collaborative Framework Using Primary Source Maps

This paper introduces the Primary Source Map (PSM), a human-AI collaborative method to mitigate hallucinations in LLMs by centering verifiable, self-issued primary sources. Built upon the Quantum-Bio-Hybrid (QB-H) theory, PSM achieved a 75% reduction in hallucination rate through provenance-based routing and human verification habits.
