D1.1 – Roadmap on navigating the complexities of enabling innovative technologies in transitional care

Description

This deliverable serves as a foundational step in the Evolve2Care project that aims at advancing innovation in transitional care through the identification and analysis of key drivers and barriers.

Transitional care, which bridges the gap between healthcare settings and home-based recovery, faces unique challenges stemming from legal, regulatory, fiscal, technical, and operational complexities. These factors often impede the development and adoption of innovative technologies essential for improving patient outcomes. 

This deliverable addresses these challenges by conducting a two-phase research effort. The first phase focuses on gathering insights through literature reviews, surveys, and stakeholder interviews, to map the landscape of enablers and obstacles to innovation. The second phase builds on this foundation, incorporating findings from the execution of real-world Use Cases that will take place within Living Labs, addressing the needs of Innovators, to deepen the analysis and validate the initial results.

With the term Use Cases we refer to the selected mini-consortia of Living Labs – Innovators that will be selected through the project Open Calls. By synthesising these insights into a comprehensive final report, Deliverable 1.1 aims to provide actionable knowledge and a strategic roadmap to support innovators, policymakers, and stakeholders in overcoming barriers and fostering a resilient, patientcentric healthcare system.
