Ilizarov apparatus in treatment of multiple fractures of the humerus

Abstract (English)

INTRODUCTION: Treating multiple fractures of the humeral diaphysis аге not generally adopted in the world. Usage of non-operational and operational treatment methods of the same multiple, closed fracture of the upper arm аге justified. Treatment technique is mostly determined Ьу the general position of the institution where the patient is treated as well as the personal experience of the surgeon.

OBJECТIVES: Was to evaluate postoperative results of multiple closed fractures of the humeral diaphysis (taking into account segmental and open fractures) using the transosseous osteosynthesis with the llizarov apparatus.

METHODS: ln Clinic for orthopedic surgery and traumatology in Novi Sad, in period from 2006 to 2013, 48 patients with various types of humeral fractures were treated. Of that number, 40 of them (83%) had closed fractures (10 segmental and З0 multiple) while 8 patients (17%) had open fractures (4 Gustilo Anderson two type I and 4 Gustilo-Anderson type 11). 26 patients (56%) were males and 22 (44%) females. The youngest patient was 18 and the oldest 64 years old (average age was 41. Right upper arm fractures were present in 31 patients (65%), fractures of the left in 17 patients (35%). Average wearing of the apparatus was 3.5 months (3-5).

RESULTS: Complete recovery was achieved within 42 patients (87%). We have extended recovery in 4 patients (9%), pseudarthrosis in two patients (4%). We had following complications: in 7 pin-site infections, which have been successfully treated with antiЬiotics and frequent bandaging, 4 transientradial nerve lesions, а transient outage of sensitive functions of ulnar nerve and опе iatrogenic pseudoaneurysm of brachial artery. То present our functional results we used Stewart-Hundley scale and based оп it, we had 38 excellent, 6 good and 4 bad results. 

CONCLUSION: Using transosseous osteosynthesis with the llizarov apparatus in treatment of multiple fractures of the upper arm diaphysis, including segmental and ореп fractures, is bу observations of the authors, the sovereign method ог the method of choice.
