Description
This is an example of a Sample/Data Access Request form, based on a similar version that was done for the CLUSTER consortium and can be used as a starting point for creating one for your project.
By referring to data catalogues and data dictionaries, requesters already know what is available for access from the project, and is a stepping off point during the feasibility discussion to find out more about these samples and datasets.
