This community collects the results of the five-year H2020 Project "ATRIAL FIBRILLATION INTEGRATED APPROACH IN FRAIL, MULTIMORBID, AND POLYMEDICATED OLDER PEOPLE", started in May 2021 and funded under the H2020 SOCIETAL CHALLENGES - Health, demographic change and well-being Programme.
The prevalence of atrial fibrillation (AF) amongst Europe’s elderly population is growing. Associated with more severe strokes, AF is an abnormal heart rhythm with rapid and irregular beating. In the context of multimorbidity, improving the management of AF is vital and requires a holistic approach. In this context, the EU-funded AFFIRMO project will develop, implement and test the effectiveness of a patient-centred approach on older multimorbid AF patients in clinical practice. Specifically, the project will identify different clusters of multimorbidity in older patients with AF. It will also assess the needs of patients, caregivers and health professionals for the comprehensive management of multimorbidity including AF.
