BLOOD TRANSFUSION-RELATED PATIENT SAFETY ISSUES

This research article titled "Blood Transfusion-Related Patient Safety Issues" focuses on the risks associated with blood transfusions, including adverse reactions such as hemolytic transfusion reaction and transfusion-transmitted infections. The study emphasizes the importance of following Malaysia Transfusion Practice Guidelines, patient identification, and labeling procedures to prevent errors and ensure patient safety. The study also highlights that most of the risks associated with blood transfusions are avoidable, and healthcare staff's education on blood transfusion safety and adherence to existing guidelines are imperative to minimize near-miss events. The study concludes by suggesting the need for continuous medical education and routine assessments to maintain patient blood transfusion safety. The study used a literature review of 21 research papers and data from the Patient Safety Council of Malaysia's official report and emphasizes the importance of mandatory reporting and Root Cause Analysis (RCA) for any incident of incorrect blood component transfused (IBCT) or near miss.
