Background
: Uterine artery Doppler has shown to have a promising role in prediction of Preeclampsia, however, there still a need for further assessment of its role. Objective: To assess the value of uterine artery Doppler in prediction of preeclampsia and perinatal outcomes. Methods: An observational prospective, longitudinal clinical study conducted during a period of 22 months in our center. The study included 50 pregnant women with preeclampsia and 100 healthy pregnant women as control group, both groups almost matched for their baseline characteristics and followed up until give birth. Bilateral Doppler examination of uterine arteries was performed abdominally by expert professional radiologist. Abnormal Doppler of uterine arteries result was established for any patient who persistently presented at least mean pulsatility index (mPI) > 95 percentile, resistance index (IR) > 95 percentile or presence of unilateral or bilateral portodiastolic notch. Maternal and neonatal outcomes were reported and then compared Results: According to Doppler findings. pulsatility index and resistive index of uterine artery showed a significant good prediction of preeclampsia. The overall validity of uterine artery Doppler in prediction of preeclampsia was good with a sensitivity, specificity, accuracy, positive predictive value (PPV) and negative predictive value (NPV) of 74.3%%, 87.5%, 81.6%, 79.3% and 80.4%,, respectively. It had been found that patients with abnormal uterine artery Doppler were about 7-fold more likely to have PE (odds ratio = 7.1). Conclusion: Uterine artery Doppler was good predictor of preeclampsia at the second trimester or later with good sensitivity, specificity and accuracy. The resistive index and the pulsatility index behave similarly with respect to the prediction of preeclampsia and can be used interchangeably. Abnormal uterine artery Doppler indices were significantly associated with adverse pregnancy outcomes
