Bird migration flow visualization v2.0

Bird migration flow visualization, updated during the WG3 workshop ‘Visualisations: from show cases to production’ in 2015 by @peterdesmet and @bartaelterman.

Changeset

• Update colours for better display in presentations
• Use stable URLs for versions
• Provide bird migration altitude profiles as preprocessed csv instead of using CartoDB API
• Change asynchronous tasks to synchronous ones for easier maintenance
• Split up the code in logical parts and remove redundancy
• Allow plug and play of case studies by using settings
• Update README in accordance with Shamoun-Baranes et al. 2016 (in review)
