Tratamientos tópicos para la leishmaniasis cutánea: una revisión narrativa, con búsqueda sistemática de la bibliografía

Description
La leishmaniasis comprende un grupo de enfermedades causadas por parásitos protozoos del género Leishmania transmitidas por la picadura de un insecto vector hematófago. Este complejo grupo de enfermedades dependiendo de la especie de Leishmania infectante y del sistema inmune del hospedador, pueden causar desde lesiones cutáneas ulcerativas, lesiones muco-cutáneas desfigurantes, hasta una forma visceral causando la muerte. La principal herramienta en el tratamiento de las diferentes formas clínicas de la leishmaniasis han sido los antimoniales pentavalentes en dosis de 20 mg/kg por día, por 20 días con una eficacia cercana al 85%. Este tratamiento es doloroso y altamente tóxico, muchos pacientes no lo concluyen, lo que favorece la reactivación de la lesión, el compromiso en mucosas y la aparición de resistencia. En ese sentido, se busca que uno de los tratamientos para las lesiones cutáneas sea aplicado de forma tópica, sin embargo, hasta la fecha este tratamiento aún está en estudio. En este trabajo se ha realizado una revisión sobre los ensayos preclínicos y clínicos de tratamientos tópicos para la Leishmaniasis Cutánea en las bases de datos “Lilacs y Pubmed” teniendo 36 artículos para la revisión total y encontrando una eficiencia en la mayoría de ellos.
