Cerebrospinal fluid phosphorylated neurofilament heavy chain and chitotriosidase in primary lateral sclerosis

This study pertains to the measurement of two neurochemical biomarkers of amyotrophic lateral sclerosis (ALS), pNFH and Chit1, in the cerebrospinal fluid (CSF) of patients with ALS, patients with the rarer variant of motor neuron disease primary lateral sclerosis (PLS), and neurological controls, i.e. patients without neurodegenerative diseases of inflammatory diseases of the central nervous system. pNFH (phosphorylated neurofilament heavy chain) is a marker of axonal degeneration, while Chit1 (chitotriosidase) is a marker of microglial inflammation. We found that both biomarkers had higher CSF levels both in ALS and in PLS in comparison to controls. More interestingly, PLS showed levels of both biomarkers which were intermediate between ALS and controls. Therefore our study shows that pNFH and Chit1 not only are useful for the diagnosis of ALS, but are also promising for the diagnosis of PLS and especially for its differentiation from ALS.
