Psychological Recovery and Return to Sport After Injury

Returning to sport after injury requires more than restoration of physical capacity. Athletes must interpret bodily signals, renegotiate their relationship with sport, and decide whether participation remains both desirable and acceptable. This critical narrative review examines athletic identity, fear of reinjury, and psychological readiness, using anterior cruciate ligament reconstruction as the principal empirical context. Targeted searches of publicly accessible research records, publisher pages, and reference lists identified conceptual, observational, qualitative, measurement, and intervention studies. The synthesis distinguishes evidence about returning to sport from evidence about subsequent injury and treatment effectiveness. Three findings organize the analysis. Athletic identity can remain stable, weaken, or strengthen during rehabilitation; a declining group mean therefore does not describe every athlete. Fear encompasses consequences for everyday life and future participation, and its significance depends on the activity and recovery stage. Psychological readiness is associated with return, but its relationship with subsequent injury differs across samples and assessment times. Recent randomized evidence also cautions against assuming that a plausible psychological intervention will improve return rates. The review develops an interpretive framework in which physical capacity, psychological willingness, and opportunity for supported participation are examined separately and then considered together. Its contribution is a set of testable propositions about confidence calibration, identity flexibility, and decision quality. Psychological care should support informed participation and personally meaningful recovery, while its effects on sustained performance and reinjury require direct evaluation.
