Platelet-Rich Plasma with Microneedling in PCOD Associated Alopecia: A Case Report

Abstract
Introduction: Female pattern hair loss associated with polycystic ovarian disease (PCOD) is commonly linked to hyperandrogenism and may progress to advanced stages such as Ludwig Grade III.
Management of severe cases remains challenging. Platelet-rich plasma (PRP) combined with microneedling has emerged as a regenerative treatment modality to enhance follicular stimulation and dermal remodelling.
Methods: A 26-year-old female presented with a two-year history of progressive hair shedding and thinning and had a confirmed history of PCOD. Clinical examination revealed Ludwig Grade III alopecia. The treatment protocol consisted of four sessions of autologous PRP combined with microneedling using a Dermapen device at two-week intervals. PRP was prepared using a standardized double-spin technique. A vitamin-rich dietary regimen was advised concurrently.
Evaluation was performed at baseline and every two weeks using global photography, trichoscan analysis, and a 60-second hair count test.
Results: Progressive reduction in hair shedding and improvement in hair density were observed. Trichoscananalysis demonstrated increased terminal hair growth and improved follicular density. No adverse effects were reported.
Conclusion: PRP with microneedling appears to be a safe and effective modality for advanced PCODassociated alopecia.
