Description

StratifyHF is an AI-based Decision Support System designed to enable early diagnosis, risk stratification, and progression prediction of heart failure (HF) in both primary and secondary care settings. The project integrates patient-centered data collected through existing and novel technologies with a digital patient library, combining computational mechanics - including multiscale, multiphysics finite element modeling - with artificial intelligence algorithms to model and predict disease progression. This Research and Innovation Action is funded by the European Union's Horizon Europe programme (Grant Agreement No. 101080905) and coordinated by the Bioengineering Research and Development Center (BioIRC), Kragujevac, Serbia, together with an international consortium of partners. This work, presented at the 7th International Conference on Multi-scale Computational Methods for Solids and Fluids (MSF 2025, Split, Croatia), describes the computational and AI-based methods developed within StratifyHF for heart failure risk prediction and decision support.
