Abstract: Pregnancy is characterized by profound physiological adaptations involving cardiovascular, respiratory, immunological, endocrine, metabolic, hematological, and placental systems that collectively support fetal growth and maternal well-being. The coronavirus disease 2019 (COVID-19) pandemic introduced unprecedented challenges to maternal–fetal health and generated substantial scientific interest regarding the impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on pregnancy physiology. Although the acute phase of the pandemic has subsided in many regions, the post-COVID-19 era continues to reveal important insights into maternal–fetal adaptations, long-term health consequences, placental alterations, immune programming, and pregnancy outcomes. Emerging evidence suggests that COVID-19 may influence maternal cardiovascular function, endothelial health, immune responses, placental physiology, and fetal development, with implications extending beyond the immediate infection period. Furthermore, advances in vaccination, telemedicine, prenatal surveillance, and precision medicine have transformed obstetric practice and improved understanding of pregnancy-related physiological responses. This review synthesizes current evidence regarding maternal–fetal physiological adaptations in the post-COVID-19 era, highlights lessons learned from pandemic-related research, and discusses future directions for clinical management and scientific investigation. Understanding these evolving adaptations is essential for optimizing maternal and neonatal outcomes and strengthening preparedness for future infectious disease challenges.
