El síndrome de Miller-Fisher (SMF) es una variante infrecuente del espectro de Guillain-Barré, caracterizada por la tríada de oftalmoplejía, ataxia y arreflexia. Aunque su evolución suele ser benigna, algunas formas graves pueden comprometer el sistema nervioso autónomo.
Este artículo presenta el caso de un paciente de 56 años con SMF confirmado por anticuerpos anti-GQ1b, que desarrolló disautonomía severa con hipotensión ortostática y bradicardia sintomática, requiriendo soporte vasopresor y marcapasos temporal.
Se realizó una búsqueda narrativa de literatura en PubMed y otras bases, identificando tres reportes clínicos relevantes con diferentes presentaciones: casos leves tratados con inmunoglobulina intravenosa, uno durante el embarazo con plasmaféresis. La comparación con estos casos destaca la diversidad fenotípica del SMF y la importancia de un abordaje multidisciplinario. En el caso reportado, el tratamiento con IVIG, soporte hemodinámico y rehabilitación intensiva condujo a una recuperación funcional superior al 90 % en cuatro semanas.
Se enfatiza la necesidad de protocolos clínicos que incluyan evaluación autonómica y monitoreo cardiovascular en SMF severo.

Miller-Fisher syndrome (MFS) is a rare variant within the Guillain-Barré spectrum, classically defined by the triad of ophthalmoplegia, ataxia, and areflexia. While most cases have a favorable prognosis, some severe forms may affect the autonomic nervous system.
This article reports a 56-year-old male with MFS confirmed by anti-GQ1b antibodies who developed severe dysautonomia, including orthostatic hypotension and symptomatic bradycardia, requiring vasopressor support and temporary pacemaker implantation.
A narrative literature review using PubMed and other databases identified three relevant case reports, including mild cases treated with IVIG and one during pregnancy managed with plasmapheresis. Comparison highlights the clinical heterogeneity of MFS and underscores the importance of a multidisciplinary approach. In our patient, early immunotherapy, hemodynamic stabilization, and intensive rehabilitation achieved over 90% functional recovery within four weeks.
The case supports the development of clinical protocols that incorporate autonomic assessment and cardiovascular monitoring in severe MFS presentations.
