This white paper is the third publication in the Understanding Recurrent and Chronic Cystitis series by the European Cystitis Observatory (ECO). It explores different manifestations of postcoital cystitis (PCC) and the role of the pelvic floor in the onset, persistence and progression of the condition.
The report is based on the Dimann dataset, a large collection of anonymised, patient-reported questionnaires originally gathered during the Dimann product-assignment process. While the dataset was not initially designed to answer specific research questions, it provides a unique opportunity for exploratory, real-world insights into the clinical, behavioural, diagnostic, pelvic and quality-of-life dimensions of recurrent and chronic cystitis.
The overall dataset includes 34,000+ valid observations collected between 2022 and 2025 across four European countries (Italy, France, Germany and Spain). For this publication, a dedicated sub-sample of 22,716 women with postcoital cystitis was identified through a multi-criterion procedure combining self-reported cystitis type, recognition of sexual intercourse as a trigger, and timing of symptom onset after intercourse.
Key findings suggest that postcoital cystitis is not a single, uniform condition. Cluster analysis identified three clinically coherent profiles. The paper highlights a major diagnostic gap: many women recognise the link between sexual intercourse and cystitis without knowing the clinical name of the condition or receiving a targeted preventive strategy.
The publication also discusses the pelvic floor as a central but often neglected mechanism in the vicious cycle of postcoital cystitis. Pelvic floor hypertonia, dyspareunia, vestibular pain, recurrent inflammation and fear of intercourse can reinforce one another, contributing to recurrence and symptom persistence. The findings support a shift from a uniform, antibiotic-centred approach to differentiated, multimodal and patient-centred pathways, integrating microbiological diagnosis, non-antibiotic strategies, pelvic floor physiotherapy, hormonal support and psychological or psychosexual care where appropriate.
