EDITORIAL

La insuficiencia cardíaca ha experimentado una transformación conceptual y terapéutica significativa, consolidándose como un síndrome cardiometabólico sistémico que requiere un abordaje individualizado basado en el fenotipo clínico, el nivel de riesgo y el momento terapéutico. Este editorial revisa los principales avances que han marcado la práctica clínica contemporánea, destacando la incorporación de los inhibidores del cotransportador sodio-glucosa tipo 2 (iSGLT2) a lo largo de todo el espectro de la enfermedad, el papel emergente de los agonistas del receptor GLP-1 en pacientes con insuficiencia cardíaca con fracción de eyección preservada y obesidad, la optimización del uso de vericiguat en pacientes de alto riesgo y el avance de estrategias de medicina de precisión en fenotipos específicos. Asimismo, se enfatiza la importancia de una interpretación adecuada de biomarcadores, la consideración de factores cardiometabólicos y la necesidad de intervenciones tempranas y personalizadas. El manejo actual de la insuficiencia cardíaca se orienta hacia una medicina más precisa, centrada en seleccionar el tratamiento adecuado para cada paciente en el momento oportuno, con el objetivo de mejorar el pronóstico y reducir eventos clínicos adversos.

Abstract (English)

EDITORIAL:

Heart failure has undergone a major conceptual and therapeutic transformation, becoming recognized as a systemic cardiometabolic syndrome that requires an individualized approach based on clinical phenotype, risk profile, and therapeutic timing. This editorial reviews the most relevant advances shaping contemporary clinical practice, highlighting the widespread incorporation of sodium-glucose cotransporter-2 inhibitors (SGLT2 inhibitors) across the entire spectrum of heart failure, the emerging role of GLP-1 receptor agonists in patients with heart failure with preserved ejection fraction and obesity, the optimization of vericiguat use in high-risk patients, and the progress of precision medicine strategies in specific phenotypes. The article also emphasizes the importance of appropriate biomarker interpretation, the integration of cardiometabolic factors, and the need for early and personalized interventions. Contemporary heart failure management is evolving toward a more precise and phenotype-guided model, focused on delivering the right treatment to the right patient at the right time in order to improve prognosis and reduce adverse clinical outcomes.
