The incidence of monozygotic (MZ) twinning in Assisted Reproductive Technology (ART) is significantly higher than in natural conception, posing severe maternal and fetal risks, including Twin-to-Twin Transfusion Syndrome (TTTS). This paper shifts the paradigm of MZ twinning from an embryological "random event" to an epigenetic stress response. We hypothesize that oxidative stress, zona pellucida glycation via advanced glycation end products (AGEs), and compromised sperm DNA/centrosomal integrity disrupt E-cadherin function and inner cell mass (ICM) cohesion. To mitigate these triggers, we introduce the Predictive Reproductive Management Protocol for Twinning (PRMP-Twin™), a three-phase clinical algorithm designed to stabilize the pre-conceptional environment (in vivo), optimize embryological handling (in vitro), and provide predictive placental monitoring using microRNA biomarkers.
