Description

Background:
Endometriosis associated with ovarian chocolate cysts is a common cause of dysmenorrhea and infertility. Recurrence after surgical management is frequent and remains a therapeutic challenge.

Casepresentation:
A 28-year-old married female presented with severe dysmenorrhea and inability to conceive. Ultrasonography revealed bilateral ovarian chocolate cysts. The patient previously underwent laparoscopic cyst excision with adhesiolysis and appendicectomy followed by ovulation induction and intrauterine insemination, which failed. Recurrence of right ovarian chocolate cyst (29×22 mm) was detected after 10 months.

Intervention:
The patient underwent Ayurvedic management including Virechana karmaLekhana basti, and internal medications such as Varunadi Kashaya, Chandraprabha Vati, and Endotone tablets.

Outcome:
Post-treatment ultrasonography showed regression of the cyst (28×21 mm). The patient conceived spontaneously after continuation of shamana therapy and is currently 5 months pregnant without complications.

Conclusion:
Ayurvedic Panchakarma and internal medications may help in reducing symptoms, improving reproductive health, and facilitating conception in patients with recurrent ovarian endometrioma.
