Esta carta al editor aborda la calidad de la atención en pacientes con diabetes mellitus tipo 2 que no se encuentran integrados en programas de manejo de enfermedades crónicas en contextos rurales. A partir de una aproximación descriptiva desde la atención primaria, se analizan diversas barreras estructurales, organizativas y asistenciales que afectan la continuidad del cuidado y los resultados clínicos de esta población. Asimismo, se destaca la importancia de fortalecer los sistemas de atención primaria mediante estrategias centradas en el paciente, el seguimiento activo y la evaluación continua de la calidad asistencial.
