This archive contains the R Markdown analysis code used to reproduce all statistical analyses, figures, and tables presented in the manuscript.

The underlying de-identified individual participant data and a data dictionary will be made available to qualified researchers upon reasonable request, subject to institutional data-sharing agreements and applicable regulatory requirements.
