Description
Long-term memory systems require safety decisions that are explainable, auditable, and comparable across deployments. Ad-hoc "free text" explanations are brittle and hard to aggregate. This anchor defines a stable reason-code taxonomy and a minimal audit event field set for anti-abuse memory architectures. The spec is designed to be safe to disclose publicly while supporting reproducible evaluation, governance, and incident response.
