Introdução
O Lúpus Eritematoso Sistêmico (LES) é uma doença autoimune complexa que, além dos sintomas clássicos, pode apresentar manifestações neuropsiquiátricas que reduzem a qualidade de vida dos pacientes. A heterogeneidade dos sintomas torna desafiadores diagnóstico e tratamento.

Objetivo
Esta revisão integrativa tem como objetivo identificar as manifestações neuropsiquiátricas mais comuns do LES e as abordagens terapêuticas recomendadas.

Método
A pesquisa foi realizada por duas pesquisadoras cegadas e independentes nas bases de dados PubMed, LILACS, SciELO, ScienceDirect e BVS, utilizando a expressão booleana:
("Systemic Lupus Erythematosus" AND ((neurological OR psychiatric) AND complications) AND treatment).

Foram incluídos ensaios clínicos, estudos observacionais, relatos de caso, revisões sistemáticas e integrativas publicados entre 2014 e 2024. Foram excluídos trabalhos em idiomas não acessíveis às revisoras, revisões de literatura sem dados originais, estudos com metodologia inadequada e artigos apenas sobre manifestações físicas do LES ou em comorbidade com outras síndromes.

De 318 trabalhos encontrados, 16 foram considerados na revisão.

Resultados
As manifestações neuropsiquiátricas mais comuns do LES incluem convulsões, cefaléia, AVE, disfunções cognitivas, ansiedade e depressão. A heterogeneidade dos sintomas exige abordagem multidisciplinar para detecção precoce, maior adesão e melhor prognóstico.

Conclusão
Pacientes com LES têm alto risco de desenvolver condições neuropsiquiátricas e apresentar sintomas mais graves. As principais manifestações neurológicas incluem convulsões, cefaléia e acidentes vasculares encefálicos (AVE). Disfunções cognitivas são recorrentes. Ansiedade e depressão são os transtornos mentais mais comuns.

O diagnóstico de neurolupus continua sendo desafiador devido à falta de critérios específicos ou exames padronizados. A psicose lúpica é rara e deve ser diferenciada da psicose induzida por esteroides. Sintomáticos como antiepilépticos, antipsicóticos, antidepressivos e ansiolíticos são frequentemente prescritos.

Terapia imunossupressora, glicocorticóides, imunoterapia sequencial, anticorpos monoclonais (rituximab), plasmaférese e transplante de células-tronco hematopoéticas devem ser considerados. O diagnóstico precoce é fundamental para um prognóstico favorável. Abordagem multidisciplinar favorece a adesão ao tratamento.

Neuropsychiatric Manifestations of Systemic Lupus Erythematosus

Introduction
Systemic Lupus Erythematosus (SLE) is a complex autoimmune disease that, in addition to its classical symptoms, may present neuropsychiatric manifestations that significantly reduce patients’ quality of life. The heterogeneity of symptoms makes diagnosis and treatment particularly challenging.

Objective
This integrative review aims to identify the most common neuropsychiatric manifestations of SLE and the recommended therapeutic approaches.

Methods
The review was conducted independently and blindly by two researchers using the PubMed, LILACS, SciELO, ScienceDirect, and Virtual Health Library (BVS) databases. The following Boolean expression was applied:
("Systemic Lupus Erythematosus" AND ((neurological OR psychiatric) AND complications) AND treatment).

Clinical trials, observational studies, case reports, systematic reviews, and integrative reviews published between 2014 and 2024 were included. Studies published in inaccessible languages, narrative reviews without original data, studies with inadequate methodology, and articles addressing only physical manifestations of SLE or comorbid syndromes were excluded.

Out of 318 identified studies, 16 were included in the final analysis.

Results
The most common neuropsychiatric manifestations of SLE include seizures, headache, stroke, cognitive dysfunction, anxiety, and depression. Symptom heterogeneity requires a multidisciplinary approach to ensure early detection, improved treatment adherence, and better prognosis.

Conclusion
Patients with SLE are at high risk of developing neuropsychiatric conditions and more severe symptoms. The main neurological manifestations include seizures, headache, and cerebrovascular accidents. Cognitive dysfunction is frequent, while anxiety and depression are the most common psychiatric disorders.

The diagnosis of neuropsychiatric lupus remains challenging due to the lack of specific criteria or standardized diagnostic tests. Lupus psychosis is rare and must be differentiated from steroid-induced psychosis. Symptomatic treatments such as antiepileptics, antipsychotics, antidepressants, and anxiolytics are commonly prescribed.

Immunosuppressive therapy, glucocorticoids, sequential immunotherapy, monoclonal antibodies (rituximab), plasmapheresis, and hematopoietic stem cell transplantation should be considered. Early diagnosis is essential for a favorable prognosis, and a multidisciplinary approach enhances treatment adherence.
