Background: Phacoemulsification has revolutionized cataract surgery globally, becoming the gold standard due to its superior outcomes and rapid recovery. In Sub-Saharan Africa, the evolution of cataract surgery has progressed from traditional techniques like couching and intracapsular cataract extraction (ICCE), to extracapsular cataract extraction (ECCE), manual small incision cataract surgery (MSICS), and now to phacoemulsification. However, adopting phacoemulsification in the region is met with unique challenges, including resource constraints, limited access to training, and patient-related factors. Objective: This guide aims to equip novice surgeons with practical skills, techniques, and insights necessary for mastering phacoemulsification, addressing the complexities, common pitfalls, and key strategies for successful transition within Sub-Saharan Africa. Methodology: The authors documented their structured training journey in transitioning from MSICS to phacoemulsification. The guide details the challenges faced, training modalities, and strategies for overcoming obstacles to achieve competence in phacoemulsification. Results: Key pathways to proficiency are outlined, covering preoperative planning, step-by-step surgical techniques, intraoperative complication management, and postoperative care. The guide emphasizes the importance of structured training programs, effective mentorship, continuous learning, and ethical practice for skill development. Conclusion: Phacoemulsification in Sub-Saharan Africa offers both challenges and opportunities for novice surgeons. With structured training, mentorship, and a commitment to continuous learning, surgeons can improve outcomes, reduce complications, and promote wider adoption of this advanced technique, ultimately enhancing patient care.
