De Garengeot's hernia – a rare clinical case and literature review

De Garengeot’s hernia represents an extremely rare form of femoral hernia with incarceration of the appendix within the hernial sac. The clinical presentation is nonspecific, which accounts for the high rate of intraoperative diagnosis. We present a 74-year-old female patient with a painful, irreducible mass in the right inguino-femoral region and signs of peritoneal irritation. Intraoperatively, a gangrenous appendix with perforation and fibrinopurulent peritonitis was found. Appendectomy and McVay repair were performed. De Garengeot’s hernia requires early recognition and appropriate surgical management.
Abstract (English)
Хернията на De Garengeot представлява изключително рядка форма на феморална херния с инкарцерация на апендикс в херниалния сак. Клиничната симптоматика е неспецифична, което обуславя висок процент интраоперативни диагнози. Представяме пациентка на 74 години с болезнена нерепонируема формация в дясна ингвино-феморална област и данни за перитонеално дразнене. Интраоперативно се установи гангренозно изменен апендикс с перфорация и фибринозно-гноен перитонит. Извършени бяха апендектомия и пластика по McVay. Хернията на De Garengeot изисква ранно разпознаване и адекватна хирургична тактика.
