Perioperative antibiotic therapy in acute appendicitis
Description
The acute appendicitis is the most common surgical emergency in the abdominal surgery, which includes the antibiotic therapy as a significant part of the complex approach in its treatment. In the modern practice it is administered both as perioperative prophylaxis and as therapeutic measure in complicated forms. The present clinical studies and international guidelines emphasize the significance of the rational choice of antibacterial regimens, the adequate time of adminstration, and the optimal duration of the therapy, while the aim is achievement of balance between efficacy and limitation of the risk from resistance. This review has as its purpose presenting of the current proofs and consensus recommendations regarding the role of perioperative antibiotic therapy in acute appendicitis.

Острият апендицит е най-честата хирургична спешност в абдоминалната хирургия, при която антибиотичната терапия заема съществено място в комплексния подход. В съвременната практика тя се прилага както като периоперативна профилактика, така и като терапевтична мярка при усложнени форми. Наличните клинични проучвания и международни ръководства подчертават значението на рационалния избор на антибактериални режими, адекватното времево приложение и оптималната продължителност на терапията, като се цели постигане на баланс между ефикасност и ограничаване на риска от резистентност. Настоящият обзор има за цел да представи актуалните доказателства и консенсусни препоръки относно ролята на периоперативната антибиотична терапия при остър апендицит.
