Description
El síndrome de Poland es una anomalía congénita rara, caracterizada por hipoplasia o ausencia del músculo pectoral mayor y diversas alteraciones musculoesqueléticas y torácicas. Las manifestaciones cardiovasculares son poco frecuentes y suelen limitarse a la dextrocardia. Se presenta el caso de un hombre de 68 años, afrodescendiente y de origen colombiano, con síndrome de Poland izquierdo, dextroposición y cardiomiopatía dilatada, quien desarrolló arritmias ventriculares malignas refractarias al tratamiento médico, ablación, bloqueo simpático y desfibrilador implantable. La dextroposición en estos casos es motivo de estudio, donde la secuencia de interrupción del flujo sanguíneo de la arteria subclavia es una de las teorías más plausibles, sin embargo, existen otras asociadas con menos sustento. El implante del dispositivo representó un reto técnico por la ausencia del pectoral mayor, debiendo situarse en tejido celular subcutáneo sin protección muscular. Pese a múltiples intervenciones, el paciente falleció por arritmias ventriculares refractarias.
Abstract (English)
Poland syndrome is a rare congenital anomaly, characterized by hypoplasia or absence of the pectoralis major muscle and various musculoskeletal and thoracic abnormalities. Cardiovascular manifestations are uncommon and usually limited to dextrocardia. The case of a 68-year-old Afro-descendant man from Colombia, with left Poland syndrome, dextroposition, and dilated cardiomyopathy, is presented; he developed malignant ventricular arrhythmias refractory to medical treatment, ablation, sympathetic block, and implantable defibrillator. Dextroposition in these cases is a subject of study, where the sequence of interruption of the subclavian artery blood flow is one of the most plausible theories; however, there are others with less support. The device implantation represented a technical challenge due to the absence of the pectoralis major, having to be placed in subcutaneous cellular tissue without muscular protection. Despite multiple interventions, the patient died from refractory ventricular arrhythmias.
