AS INFORMAÇÕES CONTIDAS NESSE PROTOCOLO FORAM REGISTRADAS EM SAVE DRAFT ATÉ O DIA DA SUBMISSÃO DO ARTIGO COMPLETO, QUE OCORREU NO DIA 09 DE SETEMBRO DE 2025, A PARTIR DESSE MOMENTO, ELE FOI PUBLICADO PARA TORNAR-SE PÚBLICO E CITÁVEL NO PRÓPRIO ARTIGO COM REGISTRO DOI POR ESTA PLATAFORMA.

Este documento descreve o protocolo para uma revisão sistemática que visa sintetizar e analisar a evidência científica sobre os procedimentos e intervenções de enfermagem para o monitoramento, avaliação e manejo dos efeitos adversos em pacientes com neurotoxicidades associadas à imunoterapia CAR-T. A terapia CAR-T, apesar de revolucionária no tratamento de malignidades hematológicas, apresenta um perfil de toxicidade único e potencialmente fatal, como a Síndrome de Liberação de Citocinas (CRS) e a Síndrome de Neurotoxicidade Associada a Células Efetoras Imunes (ICANS).

A revisão seguirá as diretrizes PRISMA2020, com uma síntese narrativa orientada pela diretriz SWiM (Synthesis Without Meta-analysis), ambas presentes na REDE EQUATOR, além disso, será aplicada a Análise de Conteúdo de Laurence Bardin. As buscas ocorrerão nas bases de dados PubMed, Web of Science, Scopus, Embase e BVS. O estudo será guiado pela estratégia PICO para auxílio na formulação da questão norteadora da pesquisa, focando em pacientes submetidos à terapia CAR-T (População), intervenções de enfermagem para manejo de toxicidades (Intervenção) e desfechos relacionados à segurança do paciente e eficácia do manejo (Desfecho).

O objetivo foi consolidar as melhores práticas, padronizar protocolos de cuidado e fornecer um guia robusto para a enfermagem oncológica, preenchendo uma lacuna na literatura, especialmente no contexto da língua portuguesa.

O protocolo foi pré-registrado na plataforma ZENODO (DOI: 10.5281/zenodo.17082064) para garantir sua transparência metodológica.

FERRAMENTAS UTILIZADAS DURANTE O PROCESSO DE DESENVOLVIMENTO DO ARTIGO

  • Software Gemini 2.5 Pro Advanced (Versão premium): foi utilizado dia 28 de agosto de 2025 para a produção de uma imagem (figura) de número 1, para ser inserida na introdução do artigo, ilustrando como é um Receptor de Antígeno Quimérico e seus domínios. O título, legenda e nota da imagem que descrevem o que ela mostra e qual foi o processo de produção da mesma (utilização exata do prompt de comando integral), podem ser visualizados no próprio artigo quando estiver disponibilizado na íntegra. A imagem, após salva em sua versão integral, foi editada no software Canva para melhor visualização da mesma.
  • Software Rayyan (Versão gratuita): foi utilizado durante o processo de coleta de dados e inclusão/exclusão de artigos para o desenvolvimento da presente revisão sistemática, esse software foi escolhido por sua facilidade de uso em grupo (mais de 1 pesquisador utilizando simultaneamente), para que o processo de seleção de estudos e coleta dos dados fosse acelerado.
  • Software Canva (Versão premium): foi utilizado dia 29 de agosto de 2025 para a produção da imagem (figura) de número 2, para ser inserida nos resultados do artigo, ilustrando como foi a seleção da amostra dos estudos (fluxograma PRISMA2020). Esse software também foi utilizado para editar e personalizar os todas as imagens presentes no artigo, para melhor visualização dos mesmos.
  • Software MermaidChart (Versão gratuita): foi utilizado dia 29 de agosto de 2025 para a produção das imagens (figuras) de números 3, 4 e 5, foram insediras na discussão do artigo, em formatos de diagramas. Esse software funciona por meio de códigos de programação, onde os mesmos podem ser visualizados em sua totalidade no arquivo em PDF anexado a essa publicação como material suplementar. Abstract (Portuguese)

A Imunoterapia CAR-T representa uma revolução inovadora no tratamento de malignidades hematológicas, embora seu sucesso seja contrabalançado por um perfil de neurotoxicidade único e potencialmente fatal, notadamente a Síndrome de Liberação de Citocinas (CRS) e Síndrome de Neurotoxicidade Associada a Células Efetoras Imunes (ICANS). Objetivou-se sintetizar e analisar sistematicamente as evidências científicas concerne procedimentos e intervenções de enfermagem para o monitoramento, avaliação e manejo dos efeitos adversos em pacientes que apresentem neurotoxicidades associadas a imunoterapia CAR-T. Realizou-se uma revisão sistemática seguindo diretrizes do PRISMA2020, com síntese narrativa orientada pela diretriz SWiM (Synthesis Without Meta-Analysis) e realizada Análise de Conteúdo de Bardin. As buscas ocorreram nas bases de dados PubMed, Web of Science, Scopus, Embase e BVS. Após aplicados critérios de inclusão e exclusão, 44 documentos foram utilizados na presente revisão. A pesquisa demonstra que a enfermagem especializada é fundamental na segurança do paciente, atuando na detecção precoce das neurotoxicidades através de monitoramento rigoroso e da aplicação de ferramentas padronizadas, como o escore ICE para avaliação neurológica. As intervenções de enfermagem abrangem desde cuidados de suporte até a administração de terapias de resgate, como Tocilizumabe e Corticosteroides, e o escalonamento do cuidado para a UTI. Além da fase aguda, o papel da enfermagem é vital na gestão de complicações tardias, como a Hipogamaglobulinemia e a Aplasia de Células B, coordenando planos de sobrevivência e educação do paciente. A conclusão reforça que a Enfermagem de Práticas Avançadas é um componente indispensável para mitigar a morbimortalidade, visando garantir o sucesso da imunoterapia CAR-T.

Abstract (English)

CAR-T immunotherapy represents an innovative revolution in the treatment of hematological malignancies, although its success is counterbalanced by a unique and potentially fatal neurotoxicity profile, notably Cytokine Release Syndrome (CRS) and Immune Effector Cell-Associated Neurotoxicity Syndrome (ICANS). The objective was to systematically synthesize and analyze the scientific evidence concerning nursing procedures and interventions for the monitoring, assessment, and management of adverse effects in patients presenting with neurotoxicities associated with CAR-T immunotherapy. A systematic review was conducted following the PRISMA2020 guidelines, with a narrative synthesis guided by the SWiM (Synthesis Without Meta-Analysis) guideline, and Bardin's Content Analysis was performed. The searches were carried out in the PubMed, Web of Science, Scopus, Embase, and VHL databases. After applying inclusion and exclusion criteria, 44documents were used in this review. The research demonstrates that specialized nursing is fundamental to patient safety, acting in the early detection of neurotoxicities through rigorous monitoring and the application of standardized tools, such as the ICE score for neurological assessment. Nursing interventions range from supportive care to the administration of rescue therapies, such as Tocilizumab and Corticosteroids, and the escalation of care to the ICU. Beyond the acute phase, the role of nursing is vital in managing late complications, such as Hypogammaglobulinemia and B-Cell Aplasia, by coordinating survivorship plans and patient education. The conclusion reinforces that Advanced Practice Nursing is an indispensable component to mitigate morbidity and mortality, aiming to ensure the success of CAR-T immunotherapy.

La inmunoterapia CAR-T representa una revolución innovadora en el tratamiento de neoplasias hematológicas, aunque su éxito se ve contrarrestado por un perfil de neurotoxicidad único y potencialmente fatal, destacando el Síndrome de Liberación de Citoquinas(CRS) y el Síndrome de Neurotoxicidad Asociado a Células Efectoras Inmunes (ICANS). El objetivo fue sintetizar y analizar sistemáticamente la evidencia científica sobre los procedimientos e intervenciones de enfermería para el monitoreo, evaluación y manejo de los efectos adversos en pacientes que presentan neurotoxicidades asociadas a la inmunoterapia CAR-T. Se realizó una revisión sistemática siguiendo las directrices de PRISMA2020, con una síntesis narrativa orientada por la directriz SWiM (Synthesis Without Meta-Analysis) y se aplicó el Análisis de Contenido de Bardin. Las búsquedas se realizaron en las bases de datos PubMed, Web of Science, Scopus, Embase y BVS. Tras aplicar los criterios de inclusión y exclusión, se utilizaron 44documentos en la presente revisión. La investigación demuestra que la enfermería especializada es fundamental en la seguridad del paciente, actuando en la detección precoz de las neurotoxicidades a través de una monitorización rigurosa y la aplicación de herramientas estandarizadas, como la puntuación ICE para la evaluación neurológica. Las intervenciones de enfermería abarcan desde cuidados de soporte hasta la administración de terapias de rescate, como Tocilizumab y Corticosteroides, y la intensificacióndel cuidado a la UCI. Además de la fase aguda, el papel de la enfermería es vital en la gestión de complicaciones tardías, como la Hipogammaglobulinemia y la Aplasia de Células B, coordinando planes de supervivencia y la educación del paciente. La conclusión refuerza que la Enfermería de Práctica Avanzada es un componente indispensable para mitigar la morbimortalidad, con el fin de garantizar el éxito de la inmunoterapia CAR-T.
