Description

This deliverable presents the outcomes of Task 1.3: Specification of Use Cases, Marketplace Architecture and Action Plan for M9, focused on designing a structured and scalable experimentation space to support HealthTech innovation in transitional care. It consolidates the project's strategic approach to use case and technical infrastructure development for facilitating real-world testing via Living Labs. This Deliverable serves as an inventory of the current tools, methodologies and literature that will help us build an effective and useful platform and community for connecting Innovators with Living Labs. 


The Deliverable begins by detailing the three refined Transitional Care Use Cases of the EVOLVE2CARE project that will drive the experimentation phase. These are:

  1. Hospital Discharge Management and Monitoring, a concept that involves improving continuity and safety during the transition from healthcare facilities to home or other care settings.
  2. Remote Monitoring and Home-Based Care Optimisation, supporting patient independence and clinical oversight through wearables, mobile apps, and AI-driven analytics.
  3. Inclusive Technologies for Aging and Chronic Care, enhancing accessibility, safety, and personalization of care for elderly individuals navigating multiple care environments.

Each use case integrates stakeholder needs and defines relevant evaluation domains to guide experimentation, along with the components of the KPI framework as defined in D1.2. To support matchmaking between innovators and Living Labs, the deliverable outlines the development of the Accelup platform. This digital infrastructure will facilitate structured collaboration through advanced profiling, matchmaking algorithms, knowledge exchange mechanisms, and embedded regulatory support. Both digital (AI-based recommenders and APIs) and methodological (profiling strategies, moderated matchmaking, co-creation tools) approaches are explored to ensure effective and transparent matchmaking, knowledge exchange collaboration. We also explored existing tools that can be used either by policy makers or regulators in order to make decisions, along with the potential integration of such tools in the Accelup platform. Furthermore, the implementation of ENoLL Living Lab Harmonization Framework plays a significant role in the development platform, as integrating already established frameworks and harmonized processes can enhance the tools credibility and acceptance. 

Finally, the deliverable defines a roadmap for implementation, identifying the steps and processes that will be performed throughout the EVOLVE2CARE project to maximize its impact. This Action Plan provides the foundation for the next project phase, where defined KPIs and platform features will be operationalized, tested, and validated through real-world experimentation activities. An updated version of this document will be delivered also in D1.6 EVOVLE2CARE Action Plan – Final on M19 of the project.
