SUMMARY

This document is the Monitoring and Evaluation Plan of the CARE4DIABETES (C4D) Joint Action 
(JA). It presents the theoretical framework as well as the methodology of evaluation, to be 
applied during the timeline of C4D, from February 2023 (M1) to January 2026 (M36). The 
document is the first deliverable (D3.1) of the Evaluation work package (WP3).

The objective of this deliverable is to present the general and specific objectives of C4D as agreed 
in the Grant Agreement (GA) and the methodology to monitor and evaluate their fulfilment, 
including the project activities across all WPs, data collection, and the Key Performance Indicators 
(KPIs). As regards to specific instructions on data collection, the level of detail presented in this 
document varies, as the methodology will evolve in dialogue with the other WPs. For example, 
the templates for patient level data collection are final, but qualitative data collection on 
implementation process will be specified in collaboration with WP4 (Sustainability), WP5 
(Preparatory actions), WP6 Phase I implementation (capacity building oriented) and WP7 Phase 
II implementation (after-care oriented). 

The intended audience are all members of the C4D consortium (competent authorities and 
affiliated entities), especially the WP leaders, C4D teams from implementing countries, and 
stakeholders involved in the pilot implementation and/or dissemination and scale-up.

The evaluation methodology was developed based on the Theory of Change, which is a theory 
driven framework and method in order to improve the evaluation of complex health 
interventions. A Theory of Change aims to explain how a project or intervention is understood to 
work. It helps to identify specific evaluation questions, relevant variables that should be included 
in the data collection, intermediate outcomes that can be used as markers of success in situations 
where the impacts of interest will not occur during the time frame of the evaluation, aspects of 
implementation that should be examined and potentially relevant contextual factors that should 
be addressed in the data collection and in the analysis. In addition, the RE-AIM framework was 
used to direct the evaluation according to the 5 key RE-AIM outcomes: Reach, Effectiveness, 
Adoption, Implementation, and Maintenance. While selecting data collection methods, 
previously validated questionnaires were favoured if available (e.g. EQ-5D-5L Quality of Life 
Questionnaire). For project monitoring, the ClickUp tool which WP3 team has used in an earlier 
JA will be used.

In conclusion, this document defines the framework and presents the methodology for the 
evaluation of C4D to ensure that actions are conducted with good quality and as planned and to 
verify whether the project will reach its objectives.
