Abstract (English)
Aims: Floating knee (ipsilateral fractures of the femur and tibia), аге а challenging ргоbеm to because those fractures often include comblnations of diaphyseal, metaphyseal, and intra-articular parts of femur and tibia, and аге often associated with soft tissue and vascular injuries. Stabllization and early mobllization of the patient produce best clinical outcomes. We present our experience and results with these type of fractures.
Materials and Methods: ln our clinic, we have treated 40 patients with floating knee from 2009 - 2013.The right side was involved in З0 and left side in 10 patients. There were 27 Туре 1, 6 Туре 2А and 7 Туре 2В floating knee injuries (Blake & McBryde classification). Age structure consisted of 32 men and 8 women. The mechanism of injury was road traffic accident. From that amount 32 fractures were closed and 8 open. The average age was 38 years. AII patients were walking full weight bearing on 3th postoperative day
Results: Average duration of hospitalization was about 2 weeks. 34 patients (85%) united at both fracture sites, within average union in 10 months (femur average in 12 , and with tibia in 3 ½ months). Follow-up was about 28 months. According to the Karlstrom criteria the end results were: excellent in 33 , good in 4, ассерtаЫе in 2 and роог in 1 cases. The complications were knee stiffness in 3, delayed union of tibia in 2 and superficial infection in 3 cases.
Conclusion: According to our experience transosseous osteosynthesis with the llizarov apparatus provides us with stabllization, early mobllity and full weight bearing and manipulation of bone fragments with the device without large surgical incisions, with minimal risk of infections.
