Description

La identificación microbiológica precisa es un pilar fundamental en el manejo de las enfermedades infecciosas. La tecnología MALDI-TOF MS ha revolucionado este campo, permitiendo una identificación rápida y confiable de microorganismos a nivel de género y, en muchos casos, a nivel de especie. Sin embargo, la relevancia clínica de esta información puede variar según el microorganismo y el contexto clínico específico.

En muchas situaciones, informar a nivel de género es suficiente desde el punto de vista clínico para guiar la toma de decisiones terapéuticas, especialmente cuando las características fenotípicas o genómicas entre las especies del mismo género no impactan significativamente en la elección del tratamiento. Este enfoque permite priorizar la velocidad en la comunicación de resultados y optimizar los recursos del laboratorio, sin comprometer la calidad del servicio diagnóstico.

Esta guía presenta recomendaciones específicas para la identificación e interpretación de resultados obtenidos mediante MALDI-TOF MS, destacando las limitaciones actuales y los casos en los que es adecuado informar únicamente a nivel de género. Asimismo, se incluyen consideraciones sobre la validación de datos y las pruebas complementarias necesarias para garantizar la fiabilidad de los resultados.

Este documento es el resultado de exhaustivos ensayos de verificación llevados a cabo por laboratorios de referencia, así como de una meticulosa revisión bibliográfica.

 

Abstract

Accurate microbiological identification is a fundamental pillar in the management of infectious diseases. MALDI-TOF MS technology has revolutionized this field, enabling rapid and reliable identification of microorganisms at the genus level and, in many cases, at the species level. However, the clinical relevance of this information may vary depending on the microorganism and the specific clinical context.

In many situations, reporting at the genus level is clinically sufficient to guide therapeutic decision-making, especially when the phenotypic or genomic characteristics among species within the same genus do not significantly impact the choice of treatment. This approach allows prioritizing the speed of communicating results and optimizing laboratory resources, without compromising the quality of the diagnostic service.

This guide presents specific recommendations for the identification and interpretation of results obtained using MALDI-TOF MS, highlighting current limitations and cases where it is appropriate to report only at the genus level. It also includes considerations on data validation and complementary tests necessary to ensure the reliability of the results.

This document is the result of extensive verification trials conducted by reference laboratories, as well as a meticulous literature review.
