Issues in the management of patient companions in Hospital : the case of Hospital du Mali

Résumé

L'accompagnement de malade est le fait d’amener un malade à l’hôpital et de rester à son chevet pour l’assister tout au long de son séjour pendant l’hospitalisation. Il s’agit d’un véritable défi sécuritaire dans les hôpitaux au Mali. Dans ce travail nous proposons d’étudier les avantages et les contraintes liés à l’accompagnement de malade à l’Hôpital du Mali. Nous avons colligé de façon prospective les impressions de 450 accompagnants et 60 personnels de l’Hôpital du Mali entre le 1^er janvier au 31 décembre 2021. Des questionnaires individuels ont été utilisés pour collecter les données qui ont été analysées sur SPSS-IBM.5.2. Nous avons trouvé une moyenne d’âge de 40,5±6,8 ans, compris entre 15 ans pour le plus jeune et 69 ans pour le plus âgé, 65 % des accompagnants étaient des femmes avec un sex-ratio de 1,8. Le nombre moyen d’accompagnants étaient 5±3 par malade, 60% des accompagnants pensaient que leur rôle est de faire la cuisine et faire les courses pour les examens et d’acheter les médicaments, 90% des accompagnants étaient permanents, ignoraient le règlement de l’hôpital et satisfaits des prestations de soins , 55,1% des accompagnants étaient installés dans un endroit non dédié à cet effet. 90% des agents trouvent que les accompagnants aident à la prise en charge des malades, cependant impacte négativement sur l’environnement. L’accompagnement de malade est un phénomène qui marque l’élan de solidarité malienne, cependant il reste une contrainte de grande envergure, car il n’est pas bien structuré et encadré par l’organisation pratique des soins à l’Hôpital du Mali.

Summary

Patient support is the act of bringing a patient to the hospital and staying at his bedside to assist him throughout his stay during hospitalization. This is a real security challenge in hospitals in Mali. In this work we propose to study the advantages and constraints related to the accompaniment of patients at the Hospital of Mali. We prospectively collected the impressions of 450 caregivers and 60 staff from the Mali Hospital between January 1 and December 31, 2021. Individual questionnaires were used to collect the data which was analyzed on SPSS-IBM.5.2. We found an average age of 40.5 ± 6.8 years, between 15 years for the youngest and 69 years for the oldest, 65% of the companions were women with a sex ratio of 1.8 . The average number of companions was 5 ± 3 per patient, 60% of the companions thought that their role was to cook and shop for exams and to buy medicines, 90% of the companions were permanent, were unaware of the rules of the hospital and satisfied with the care services, 55.1% of the accompanying persons were installed in a place not dedicated to this purpose. 90% of the agents find that the companions help in the care of the sick, however they have a negative impact on the environment. Patient support is a phenomenon that marks the momentum of Malian solidarity, however it remains a major constraint, because it is not well structured and framed by the practical organization of care at Hospital du Mali.
