This zip file contains the email template used to invite participants, the consent letter, the pre-test interview, the protocol for the evaluator, the participant tasks, a questionnaire, and a description of three scenarios.

Note that the Symptoms Pal application discussed in the paper was named Symptom Checker Mobile (SCM) at the time we performed the usability study.
