Introducción: la esquizofrenia es un trastorno psiquiátrico complejo caracterizado por síntomas psicóticos, alteraciones en la cognición y disfunción social. Se ha sugerido una relación entre las anomalías cerebrales estructurales y la aparición de síntomas psiquiátricos. La megacisterna magna (MCM) es una anomalía congénita de la fosa posterior del cerebro cuya relevancia clínica ha sido debatida. El objetivo es dar a conocer la relación que existe entre la esquizofrenia y algunos hallazgos imagenológicos como lo es la megacisterna magna.

Presentación del caso: en este reporte se describe a un paciente masculino de 19 años con diagnóstico de esquizofrenia refractaria y múltiples hospitalizaciones psiquiátricas, en quien se identificó una MCM como hallazgo incidental en la neuroimagen.

Discusión: se analiza la posible relación entre esta anomalía y la evolución neuropsiquiátrica del paciente, considerando factores adicionales como el consumo de sustancias y la adherencia al tratamiento.

Conclusiones: este caso destaca la necesidad de investigaciones adicionales para determinar la implicación clínica de la MCM en los trastornos psiquiátricos y su impacto en el manejo terapéutico.

Abstract (English)

Article type: Clinical case

Introduction: Schizophrenia is a complex psychiatric disorder characterized by psychotic symptoms, cognitive impairment, and social dysfunction. A relationship between structural brain abnormalities and the development of psychiatric symptoms has been suggested. Megacisterna magna (MCM) is a congenital anomaly of the posterior fossa of the brain whose clinical relevance has been debated. The objective is to present the relationship between schizophrenia and some imaging findings such as the megacisterna magna.

Case presentation: This report describes a 19-year-old male patient diagnosed with refractory schizophrenia and multiple psychiatric hospitalizations. MCM was identified as an incidental finding on neuroimaging.

Discussion: The possible relationship between this anomaly and the patient's neuropsychiatric outcome is analyzed, considering additional factors such as substance use and treatment adherence. Conclusions: This case highlights the need for further research to determine the clinical implications of MCM in psychiatric disorders and its impact on therapeutic management.
