A Relação de Patógenos Periodontais e Doenças Respiratórias: Revisão de Literatura

O microecossistema oral é um ambiente altamente complexo, formado pelo microbioma bucal, pelas estruturas anatômicas da cavidade oral, pela saliva e pelas múltiplas interações estabelecidas entre a microbiota e o hospedeiro. A dinâmica desse ecossistema desempenha papel fundamental na manutenção da saúde bucal e sistêmica, sendo crescente o interesse científico em compreender como alterações nesse equilíbrio podem repercutir em outros sistemas do organismo. Nesse contexto, esta revisão de literatura tem por objetivo investigar o que está descrito na literatura recente sobre a relação entre periodontopatógenos e doenças respiratórias, considerando especialmente a influência dos processos inflamatórios e da migração microbiana. A doença periodontal, caracterizada por um processo inflamatório crônico nos tecidos de suporte dos dentes desencadeado pelo acúmulo de biofilme bacteriano, tem sido amplamente estudada como potencial fator contribuinte para alterações pulmonares. Para este trabalho, realizou-se uma busca sistematizada de artigos científicos nas bases de dados PubMed, Google Scholar e Portal de Periódicos CAPES. A pesquisa ocorreu entre setembro e novembro de 2025, totalizando dois meses de levantamento bibliográfico, e incluiu estudos publicados entre 2019 e 2025. Como resultado, observou-se que diversos estudos apontam que a doença periodontal e os microorganismos a ela associados podem exercer influência sobre diferentes doenças respiratórias, como pneumonia, asma, COVID-19 e doença pulmonar obstrutiva crônica (DPOC). Entre os principais mecanismos propostos destacam-se a resposta inflamatória sistêmica decorrente da periodontite e a possível aspiração de bactérias orais para o trato respiratório inferior, contribuindo para agravamento ou predisposição às doenças pulmonares. Apesar das evidências emergentes, torna-se evidente a necessidade de mais estudos, com maior rigor metodológico e acompanhamento longitudinal, para que seja possível compreender, delimitar e confirmar de maneira consistente a relação direta entre periodontopatógenos e doenças respiratórias ao longo do tempo.

The oral microecosystem is a highly complex environment, formed by the oral microbiome, the anatomical structures of the oral cavity, saliva, and the multiple interactions established between the microbiota and the host. The dynamics of this ecosystem play a fundamental role in maintaining oral and systemic health, and there is growing scientific interest in understanding how alterations in this balance can affect other systems of the organism. In this context, this literature review aims to investigate what is described in recent literature about the relationship between periodontopathogens and respiratory diseases, especially considering the influence of inflammatory processes and microbial migration. Periodontal disease, characterized by a chronic inflammatory process in the supporting tissues of the teeth triggered by the accumulation of bacterial biofilm, has been widely studied as a potential contributing factor to pulmonary alterations. For this work, a systematic search of scientific articles was carried out in the PubMed, Google Scholar, and CAPES Periodicals Portal databases. The research took place between September and November 2025, totaling two months of bibliographic review, and included studies published between 2019 and 2025. As a result, it was observed that several studies indicate that periodontal disease and the microorganisms associated with it can influence different respiratory diseases, such as pneumonia, asthma, COVID-19, and chronic obstructive pulmonary disease (COPD). Among the main proposed mechanisms, the systemic inflammatory response resulting from periodontitis and the possible aspiration of oral bacteria into the lower respiratory tract stand out, contributing to the aggravation or predisposition to pulmonary diseases. Despite the emerging evidence, the need for more studies, with greater methodological rigor and longitudinal follow-up, is evident in order to understand, delimit, and consistently confirm the direct relationship between periodontopathogens and respiratory diseases over time.
