When interventions are inadequately described in systematic reviews, decision-makers must guess what to implement, how to do it, and what not to implement. Moreover, the reliability of a systematic review depends on appropriate consideration of intervention details in the planning, conduct, and reporting of systematic reviews. The following recommendations for EPOC review authors are based on Hoffmann et al. EPOC review authors should adhere to these recommendations or, if there are good reasons for not doing so, provide those reasons; e.g. for pragmatic reasons, it might not be possible for all EPOC reviews to prepare a table that summarises the intervention details for each study

The Norwegian satellite of the Effective Practice and Organisation of Care (EPOC) group supports reviews that are relevant to low- and middle-income countries (LMICs) and that are undertaken by people from LMICs. For reviews that are particularly relevant to LMICs the title registration form should specify clearly whether the review will only include studies from LMICs and, if so, why.

The vast majority of searches for EPOC reviews are conducted by one of our Information Specialists. However it remains important that authors are aware of the principles involved in accurate search reporting. To assist authors in recording and reporting search methods we have developed the following standardized language and format for reporting search methodology used in Cochrane EPOC protocols, review and updates.

Cochrane Effective Practice and Organisation of Care (EPOC)

The issue of “lumping and splitting” comes up frequently in relationship to EPOC reviews; i.e. whether the focus of a review is too narrow or too broad. For example, a review might address a broad question regarding the effects of any intervention to improve adherence to (any) guidelines by any health professional in any setting. Alternatively, a review might address a narrow question regarding the effectiveness of computerised reminders (a specific type of intervention) to improve prescribing of antibiotics by physicians in primary care. Often review authors propose an overly narrow focus reflecting their specific interest in the effects of a specific intervention targeted at a specific problem and a specific population in a specific setting. Overly narrow questions are ones that focus on subgroups of populations, interventions or settings without an adequate justification. They may also focus on subgroups of problems, such as specific types of professional practice. In effect, such reviews are subgroup analyses of a broader question. A number of problems may arise from such inappropriate subgroup analyses.

Because EPOC reviews inform decisions about interventions that affect populations of people, equity is an important consideration. Systematic reviews can sometimes help to inform equity considerations by providing decision makers information about the effects of interventions on disadvantaged populations.

EPOC has limited resources to support review authors undertaking Cochrane qualitative evidence syntheses. In addition, methodological and editorial guidance for these syntheses is still under development within the Cochrane Collaboration and elsewhere. EPOC will therefore only register a very limited number of qualitative evidence syntheses.

PDQ-Evidence (pdq-evidence.org) is a database that focuses on evidence on health systems questions. It includes systematic reviews, broad syntheses (including evidence-based policy briefs, overviews of reviews, evidence-based guidance, and other evidence-based documents synthesising systematic reviews), primary studies included in systematic reviews, and structured summaries of that evidence. It was developed and is maintained by systematically searching PubMed and other databases for relevant systematic reviews and overviews of reviews. Only primary studies that have been included in a systematic review are included in PDQ-Evidence.

Before submitting the Review Proposal Form to EPOC, we ask authors to conduct searches for both Cochrane and non-Cochrane reviews on the proposed topic. The purpose is two-fold:

1. To prevent duplication of topics within the Cochrane Database of Systematic Reviews (CDSR).

2. To identify the existing evidence-base in a topic area so that the proposed review topic builds on, but does not replicate published research.

This is a protocol for a Cochrane Review (update).

Background

Key barriers to effectively supporting adherence include poor awareness amongst healthcare professionals (HCPs), scarce clinical tools and interventions, and suboptimal patient-provider communication. A Cochrane review assessed the impact of feedback interventions amongst physicians published until 2016. Other HCPs are increasingly involved in supporting adherence.

Objectives

To assess the effects of providing healthcare providers with medication adherence feedback as a mechanism for improving patients’ medication adherence. To assess the impact of the intervention upon clinical outcomes, patient-reported outcomes, economic/financial outcomes and processes of care.

Search methods

We will search RCTs and cluster RCTs on CENTRAL, CINAHL, Embase, MEDLINE, PsychArticles and PsychInfo, and grey literature sources.

Selection criteria

Inclusion criteria are any reported intervention providing adherence feedback to HCPs as a key component, for long term medication for chronic diseases.

Data collection and analysis

One author performs title and abstract screening. Four authors will review full texts, extract data, and assess risk of bias. We will assess intervention effects on medication adherence, clinical outcomes, patient-reported outcomes, economic/financial outcomes and processes of care. We will assess heterogeneity, sensitivity, and undertake meta-analysis where appropriate.

Main results

The search yielded 4415 articles (pending grey literature). Publication is anticipated Q1 2023.

Author’s conclusions

Medication adherence feedback to HCPs could contribute significantly to reducing key barriers associated with poor adherence. The Cochrane review will provide evidence of effectiveness amongst HCPs whist updating that regarding physicians.

Explanatory factors are characteristics of populations (including their settings or contexts), interventions, the comparison, outcome measures, or study design (including the risk of bias) that could potentially explain differences in results. This may be, for example, due to different mechanisms of action. Explanatory factors can potentially explain differences in results both across studies and across subgroups within a study. They can be used to explore heterogeneity in subgroup analyses and meta-regressions.
