A five-year-old girl presented with a single erythematous, maculopapular, crusted lesion with slightly raised edges, about 5 cm in diameter, located in the palm of the hand.
It is a pruritic lesion without improvement after topical corticoid treatment.

From the Pathological Service it is ruled out the isolation due to the form and distribution of the lesions and the granuloma annulare by presenting scaling.
Finally, the diagnosis of an unknown ringworm previously treated with corticosteroids is reached, which reduce inflammation and make diagnosis difficult, maintaining the active lesion.
Antimycotic treatment was prescribed for six weeks.
