We report the case of a four-year-old Asian girl who presented with pain associated with a small erythematous tumor and increased volume in the right preauricular region of five days duration.
She reported fever from the onset of symptoms, with a maximum peak of 39 °C. No other symptoms were present.
Parents reported that the lesion had progressively increased in volume on previous days.

During his stay in the emergency department an Interconsultation is made to the Department of Otolaryngology, which defines the lesion as an infected right preauricular fistula.
Oral antibiotic treatment was initiated with amoxicillin-clavulanic acid 40 mg/kg for ten days.
