A 72-year-old woman with long-standing rheumatoid arthritis treated with corticosteroids developed swelling and pain along the 2nd and 3rd radios of the left hand for two days after a needle puncture.
Physical examination revealed pain in the flexor retinaculum of the radius 2nd and 3rd, and an attitude towards the hyperextended fingers due to probable tendinous rupture.

The colorless findings revealed a hypoechogenic area surrounding the flexor tendons along the 2nd and 3rd ray, from the synovium-symphalangeal joint to the distal flexor metastructure, a deep flexor speech.
Open drainage of large amounts of pus was performed using Brunner type volar incisions.
