A 14-year-old male with a history of seizures in early childhood with fever and congenital heart disease was admitted to hospital with a casual presence of x-ray abnormalities requested for evaluation of scoliosis.
She did not report any cardiac symptoms.

ECG showed QRS axis +120°, P wave axis +150°, narrow QRS voltage attenuated in left precordial leads, compatible with the radiological finding.
Pathogenesis was confirmed without associated anomalies in Doppler echocardiography.
Abdominal ultrasound found the liver to have normal characteristics located in the left hypochondrium with normal bile ducts and the spleen with normal appearance located in the right hypochondrium, all findings consistent with situ vascular changes, including total vascular flow.
No opolysplenia.
