A 65-year-old male presented with an infravesical obstructive syndrome and a soft mass in the right hemiscrotum, which decreased in size at home.
Physical examination revealed an enlarged right pouch and a reducible inguinal hernia.
The rectal examination revealed a prostate of medium size, bilobulated and adenomatous characteristics.
Among the complementary examinations a total PSA was performed that was normal, a vesicoprostatic ultrasound showed a prostate of 57 grams and a testicular ultrasound with an image suggestive of bladder herniation.

Finally, cystography revealed a massive bladder hernia in the right hemiscrotum.

The treatment was surgical and consisted of resection of the herniated portion of the bladder to peritoneal and correction of inguinal hernia with marlex mesh.
In a second time a prostate TUR was performed.
After six months, the patient remains asymptomatic.
