The patient had a nodular lesion on the nose that had grown gradually in the last three months.
One year before he had had two abscesses in his right arm, which were drained, and two months later he noticed a third abscess in his arm.
Physical examination revealed nodular plaques, edematous and covered by crusts, in the periorbital and right antecubital regions.
Direct examination and culture were positive for Coccidioides spp.
With the diagnosis of cutaneous candidiasis, he was treated with itraconazole 400 mg daily.
