A 22-month-old girl was seen in our hospital with a 5-minute generalized tonic-clonic seizure, not accompanied by fever.
The patient had vomiting and diarrhea lasting 2 days.
On physical examination a restless patient was found, with normal development for his age.
His temperature was 36.7°C, with 128 beats per minute, respiratory rate of 24 per minute, and 104 mmHg blood pressure.
Skin turgidity was reduced; the tone was normal.
No neurological alterations were detected.
There was no history of congenital anomalies or epileptic disease.
Laboratory studies showed: hemoglobin 13 g/dl, hematocrit 38%, mean corpuscular volume 78 fL, leukocytes 8400/mm3, platelets 43700/mm3.
The smear showed lymphocytic predominance and hypochromic erythrocytes of normal size.
Other studies were: CRP < 1 mg/dl, glucose 92 mg/dl, urea 17 mg/dl, creatinine 0.3 mg/dl, sodium 142 mEq/L, potassium 4.5 mEq/L, calcium 10.1 mg/dl, alkaline U-48).
The CSF was clear, without cells, with normal values of proteins and glucose.
There were no pathological findings on urine or fecal microscopy.
The investigation of rotavirus antigen in stool by the latex agglutination method was positive.
In the second hour of his hospitalization he suffered a tonic-clonic seizure that lasted 30 seconds.
Intravenous diazepam 0.3 mg/kg was administered.
EEG was normal.
No germs were isolated in the coprocultive.
There were no abnormal findings on brain magnetic resonance imaging performed after hospital admission.
