A 73-year-old male, with a history of asthma, under corticoid treatment, presented with pain and swelling in the Achilles regions with functional impotence of five days of evolution.
Symptoms began 48 hours after starting levofloxacin (500 mg/day) for upper respiratory tract infection.
Ultrasound showed bilateral thickening of the Achilles tendon in the central third of the tendon, preserving the tapering position.
Antibiotic treatment was suspended and the symptoms resolved within 20 days by rest and with NSAIDs.
