A 40-year-old man presented with a 4 cm diameter pyelic stone of the left kidney. He was referred for PCNL.
Under general anesthesia and in prone position the kidney is fixed by the inferior vena cava group under scopy control.
The percutaneous access route was described with Alken coaxial metallic dilators.
Stone-BreakerTM intracorporeal lithotriptor (LMA Urology, Gland, Switzerland) fragments lithiasis.
The stone fragmentation time was 30 minutes using three cartridges of compressed carbon dioxide.
There were no complications and the patient was discharged 72 hours after the procedure with a double J catheter due to the existence of stone fragments smaller than 3 mm.
