A 20-year-old male, fisherman, with no relevant medical history, complaining of pain and swelling in the index finger for two days after puncture with a fish bone.
The physical examination revealed four signs of Kanavel's syndrome (symmetrical swelling and semi-flexed finger-extension attitude, pain at palpation of the flexor tendon course of the second finger.
Ultrasound showed a hypoechogenic area around the flexor tendon of the index finger and marked synovial hypervascularization.
Surgical exploration revealed a small amount of pus within the synovial sheath through drainage opening the A1 and A5 pulleys and irrigation system.
