GFR 30-50 3 x 3-5 g
GFR <30 2-3 x 2 g
CHD 4 g after dialysis
CVVH 3 x 5 g (up to 8 g)
*in case of meningitis and life-threatening infections

CAVE:
Dissolve in distilled water, close monitoring of sodium, potassium

Advantages: Very good tissue penetration
